Notice of Privacy Practices

Humancare Connect, Inc. is a covered entity under the Health Insurance Portability and Accountability Act of 1996 (HIPAA). We are required by law to maintain the privacy and security of your protected health information (PHI), to provide you with this notice describing our legal duties and privacy practices, and to notify you in the event your unsecured PHI is involved in a breach.

Our Legal Duty

We are required to abide by the terms of the notice currently in effect. We reserve the right to change our privacy practices and to make the revised practices effective for all PHI we maintain, including PHI created or received before the change. When we make a material change, we will update this notice and make it available on our Platform. You may request a paper copy at any time.

Permitted Uses and Disclosures

We may use and disclose your PHI for treatment purposes — for example, sharing your records with the licensed provider conducting your telehealth consultation, or transmitting a prescription to your pharmacy. We may use and disclose your PHI for payment purposes, such as billing your health insurer, verifying insurance coverage, and processing claims. We may also use and disclose your PHI for healthcare operations, including quality assessment, provider performance evaluation, staff training, legal and compliance reviews, and business planning activities.

In addition to treatment, payment, and healthcare operations, we may use or disclose your PHI without your written authorization as permitted or required by law. This includes disclosures required by court order, subpoena, or lawful government request; public health reporting as required by state and federal agencies; health oversight activities such as government audits and inspections; disclosures necessary to prevent a serious and imminent threat to health or safety; and disclosures for workers' compensation claims.

Uses and Disclosures Requiring Your Authorization

Most uses and disclosures of your PHI not described above require your written authorization. This includes the sale of PHI, most marketing uses, and psychotherapy notes. You may revoke an authorization at any time in writing, though revocation will not affect disclosures already made in reliance on your prior authorization.

Your Rights

You have the right to inspect and obtain a copy of your PHI. We may charge a reasonable cost-based fee for copies and will respond within 30 days of your request. You have the right to request that we amend PHI you believe is incorrect or incomplete; we will respond within 60 days. You have the right to request an accounting of disclosures we have made of your PHI for purposes other than treatment, payment, and healthcare operations. You have the right to request restrictions on certain uses and disclosures, though we are not required to agree in all cases — except that we must agree to restrict disclosure to a health plan for services you have paid for in full out of pocket. You have the right to request that we communicate with you about your PHI using alternative means or at an alternative location. You have the right to be notified if your unsecured PHI is involved in a breach. We will notify you without unreasonable delay and no later than 60 days after discovering a breach.
